We give you a solid starting point pulled from a real consent form artists already use, so you're never staring at a blank page, but every line is yours to edit, replace, or turn off.

Instead of one open text box for "any allergies?", clients answer a proper screening checklist: heart conditions, pregnancy, blood thinners, prior ink reactions, and more, each with a clear yes/no.
